Special Open Enrollment - Medical Plans
May 1, 2018 – May 25, 2018

  • If you are currently enrolled in the City EPO, PPO, or Kaiser medical plans and you do nothing then you will be automatically enrolled in your corresponding Teamster Medical Plan. 
    • Please see below for monthly rates, breakouts, and caveats.
  • If you are currently enrolled in the City EPO:
    • If you do nothing you will automatically be enrolled in Teamsters HMO Plan. This requires you to fill out the Open Enrollment Form and pick a Primary Treating Physician.
  • If you are currently enrolled in the City PPO:
    • If you do nothing you will automatically be enrolled in the Teamsters PPO Plan.
  • If you are currently enrolled in the City Kaiser Plan:
    • If you do nothing you will automatically be enrolled in the Teamsters Kaiser Plan.
  • If you have Opted out of coverage from the City:
    • You must fill out an Open Enrollment Form and Opt-Out of Coverage. If you do not submit the Open Enrollment Form and Opt-Out of Coverage by the deadline, you will be automatically enrolled in Teamsters PPO medical Plan.

If you wish to switch plans, add dependents, or drop coverage, you must fill out, complete, and submit the Open Enrollment Form and supporting documentation to the City’s Human Resources Department, 100 Santa Rosa Ave, Room 1, before 5:00 pm on Friday, May 25, 2018.

Coverages with the Teamsters medical plans will be effective July 1, 2018.
